Background: The American diet couldn't be a better breeding ground if we intentionally designed it to grow cancer, heart disease and stroke as well as to fast-track diabetes. Our poor diets and the acidic environment they create for our blood encourage these illnesses and we could not have made a better experiment (our diet) if we designed it for that research purpose. But this isn't an experiment. This is your health and your vitality is slipping away every time you try to treat another symptom with yet another pill. Each of those little aches, pains, and subtle low-grade annoyances are "warning signals" that an imbalance or deficiency exists somewhere in your biochemistry, oftentimes years before it surfaces as a diagnosable condition. The first place to look for clues is your diet. Currently the standard American diet (or "SAD" as it is rightfully known by) has degenerated to the point where 62% of calories are from refined foods and about 26% come from animal products. Of that mere 10% that remains from unrefined plant food, half of that comes from white potato products, not exactly nutrient-rich. Americans are just not eating for energy, and the numbers to follow couldn't paint a poorer picture for a Nation slipping deeper into a nutrition debt that will eventually bankrupt the US. The diet analysis mentioned is devoid of life and energy but somehow we continue to expect dead packaged foods to sustain life. They simply can't. They only add to a growing balance sheet of debt for our health in cataclysmic proportions. Those that are profiting, in addition to the pharmaceutical companies, are those selling to a "caffeine Nation" with products that aim to "fix" our extreme fatigue epidemic. The real answer begins with what you are putting on your plate. Along with learning what to simply avoid, and adopting a few smart nutrition tweaks and corrective movement suggestions in this book, you will quickly begin to get your body back in balance..

The importance of spiritual well-being and the role of "meaning" in moderating depression, hopelessness and desire for death in terminally-ill cancer and AIDS patients has been well-supported by research, and has led many palliative clinicians to look beyond the role of antidepressant treatment in this population. Clinicians are focusing on the development of non-pharmacologic interventions that can address issues such as hopelessness, loss of meaning, and spiritual well-being in patients with advanced cancer at the end of life. This effort led to an exploration and analysis of the work of Viktor Frankl and his concepts of logotherapy, or meaning-based psychotherapy. While Frankl's logotherapy was not designed for the treatment of patients with life-threatening illnesses, his concepts of meaning and spirituality have inspired applications in psychotherapeutic work with advanced cancer patients, many of whom seek guidance and help in dealing with issues of sustaining meaning, hope, and understanding cancer and impending death in the context of their lives. Individual Meaning-Centered Group Psychotherapy (IMCP), an intervention developed and rigorously tested by the Department of Psychiatry & Behavioral Sciences at Memorial Sloan-Kettering Cancer Center, is a seven-week program that utilizes a mixture of didactics, discussion and experiential exercises that focus around particular themes related to meaning and advanced cancer. Patients are assigned readings and homework that are specific to each session's theme and which are utilized in each session. While the focus of each session is on issues of meaning and purpose in life in the face of advanced cancer and a limited prognosis, elements of support and expression of emotion are inevitable in the context of each group session. The structured intervention presented in this manual can be provided by a wide array of clinical disciplines, ranging from chaplains, nurses, palliative care physicians, to counselors, psychotherapists, social workers, graduate psychology students, psychologists and psychiatrists..

Health researchers routinely evaluate health and illness across subgroups defined by their sex, gender, ethnicity, and race. All too often, these classifications are proffered as an explanation for any differences that may be detected, for example, in access to care, frequency of disease, or response to treatment. Relatively few researchers, however, have examined what these classifications mean on a theoretical level or in the context of their own research. Assume, for example, that a researcher concludes from his or her data that African- Americans utilize certain surgical procedures less frequently than whites. This conclusion may mean little without an examination of the various underlying issues. Is there such a construct as race at all? How were whites and African-Americans classified as such? Does this finding reflect inappropriate overutilization of the specific procedures among whites or inappropriate underutilization among African-Americans? To what extent are socioeconomic status and method of payment related to the less frequent use? Are there differences in the manner in which health care providers present the various treatment options to whites and to African- Americans that could account for these differences in utilization? Are there differences in health care-seeking and health care preferences between the two groups that would explain the difference in utilization? Is the racial classification a surrogate measure for another variable that has remained unidentified and unmeasured? All too often, unfortunately, such issues are ignored or lightly dismissed with an entreaty for additional research..

I was also thinking about St. Augustine, and the brief, numbered chapters of his Confessions. I thought, at first, that each of the chapters should be addressed directly to God, as they are in that book, because then the big why of the family’s story could stay front and centre the whole time. It seemed appropriate. Except that the big why never dominated the story as it originally unfolded, and was not doing so as the story unfolded before me. Scrap St. Augustine. With their lives these brothers who had no future raised life high; in their daily routines, routine itself became holy. Can art be made from terminal disease? I took my cue from them.”

After the Nancy Cruzan case was decided by the Supreme Court in 1990, and ultimately resolved by the Courts of the State of Missouri, the decision to withhold or withdraw life-prolonging nutrition and hydration appeared to many to be as noncontroversial as decisions to refuse respirators or dialysis. Even the Catholic Church held that, although there should be a presumption in favor of providing nutrition and hydration, the patient or the patient’s surrogate could overrule this presumption, if either believed the treatment was disproportionate or burdensome.
The Schiavo case changed all that.
